How to Apply for Printing of New/Lost/Damaged Education Personnel ID Cards?

Please request service applicants to submit a request for Printing of New/Lost/Damaged Education Personnel ID Cards to ULT Staff by bringing the following requirements: Cover Letter from the Unit/Institution/Faculty or a form stating that the person concerned is indeed an Employee at USU.

  1. Service Applicants come to submit a request for New/Lost/Damaged Employee ID Card Printing with the specified conditions.

  2. ULT staff will verify the service files

  3. ULT staff will forward the application to LPSE via E-Ticketing / directly

  4. LPSE admin/staff will print the ID Card and forward it to ULT

  5. Service Applicant receives output (new ID Card)

Completion Period: 5 (five) working days.

How is University Level Leave Granted?

Service Requests come to apply for University-level Leave by bringing the specified requirements.
Requirements: Leave Application Letter signed by the relevant leader.
The following is Information regarding University-Level Leave.

a. Annual leave: work for at least 1 year continuously and the length of leave is 12 days, cannot be divided into less than 3 days, annual leave can be suspended by the authorized official for a maximum of 1 year.

b. Long leave: Civil servants who have worked for at least 6 years are entitled to long leave for 3 months, and are entitled to receive full income, except allowances.

c. Sick leave: Civil servants suffering from illness can be given a maximum of 1 year, miscarriages are given 1.5 months of leave, sick leave gets full pay.

d. Maternity leave for the first, second and third births is entitled to maternity leave, 3 (three) months long, receiving full income.

e. Leave for important reasons: entitled to leave for a maximum of 1 (one) month, can be given for reasons of death in the family, taking care of inheritance rights, carrying out a first marriage.

f. Leave outside the state's responsibility: Civil servants who work for 5 years, are given the right to 3 months' leave, which can be extended for 1 year if there are important matters. Civil servants who take leave outside the state's responsibility are exempt from their position, except for their 4th maternity.

How to Borrow an Auditorium Building?

Please, service applicants come to the Integrated Service Unit by bringing the following requirements: Auditorium Borrowing Application Letter addressed to Vice Chancellor V.

  3. The Asset Bureau Admin will issue a Certificate and forward it to ULT.

Service applicants are requested to come to the Integrated Service Unit and bring the following requirements: Application Letter for Borrowing the Pancasila Building addressed to Vice Chancellor V.

  2. ULT staff will forward the application to the Asset Bureau Unit.

  3. The Asset Bureau Admin will issue a Certificate and forward it to ULT.

How to Get Student Transfer from Another State University to USU?

Please, service applicants must submit a request for Student Transfer from another State University to USU by bringing the following requirements:

a. following the transfer of duties of parents/husband/wife who have the status of Civil Servants who carry out government duties or the student concerned has the status of a Civil Servant who carries out government duties transferred to North Sumatra.
b. following the transfer of duties of parents/husband/wife who have the status of Civil Servants who carry out government duties or the student concerned has the status of a Civil Servant who carries out government duties transferred to North Sumatra.
c. The location of the state university of origin and the place of duty of the parents/husband/wife must be the same.
d. The deadline for submitting an application with a Decree on the transfer of duties of the parents/husband/wife concerned is a maximum of 12 months after the date of the Decree on the transfer of duties.
e. The transfer student is not in a state of being subject to sanctions or dropping out of studies as stated in a letter from the Rector of the State University of origin.
f. Transfer students must have participated in academic activities at the original university, have studied for 2 (two) semesters and passed at least 25 credits, have studied for 3 semesters and passed at least 40 credits, have studied for 4 semesters and passed at least 55 credits, have studied for 5 semesters and passed at least 70 credits, have studied for 6 semesters and passed at least 85 credits, have studied for 7 semesters and passed at least 100 credits, have studied for 8 semesters and passed at least 115 credits.
g. The study load that must still be taken by transfer students is at least 30 credits.
h. The original Study Program has an accreditation rating of BAN-PT/ LAM PTKes at least the same as the accreditation rating of the study program at USU.

Please request service applicants to submit a request for Student Basic Data Correction to ULT Staff by bringing the following requirements: Student Identity Card (KTM), Resident Identity Card (KTP), Family Card (KK), Original Last Diploma.
Data changes are only in the form of name and date of birth.

How to do S1 to D3 Transfer?

Please request service applicants to apply for Transfer from S1 to D3 by bringing the following requirements:
a. Students who are unable to complete their studies in the undergraduate program (S-1) or are subject to a study dropout sanction based on the evaluation of the determination of study dropout and before the sanction is determined by the Decree of the USU Chancellor are asked to resign or apply to transfer to the diploma program (D-3).
b. Students who can transfer to the diploma program (D-3) are students who are unable to complete their studies in the undergraduate program (S-1). The USU diploma program (D-3) that accepts transfers of undergraduate students within the USU environment is detailed in the USU academic regulations

How to Get a USU Student Transfer to Another State University?

Applicants for the service can apply for USU Student Transfer to Another State University with the following provisions:
a. USU students who are registered, have a student ID number, and have participated in academic activities for at least 2 (two) semesters, can transfer from USU to another university.
b. Students who have been approved to transfer by the USU leadership and have been issued a letter of decision to withdraw/transfer from USU for any reason cannot be accepted back.

1. Service Applicants submit a Letter of Application to the Faculty and the Chancellor of the University of North Sumatra by completing the specified requirements.
2. ULT Staff will verify the service files
3. ULT Staff will provide a Chancellor's Decree regarding the transfer of USU Students to other Universities (PT).

Completion Period: 5 (five) working days after the files are received by ULT Staff.

How to Get a Drop Out (DO) Decision Letter for Students?

Please request the service applicant to submit a Student Drop Out (DO) Study Termination Decree by bringing the following requirements:
a. New students who have registered as USU students but without any clear and accountable reason do not attend lectures in semester I which should be mandatory, are automatically declared to have resigned as USU students.
b. Students who do not fulfill and carry out academic obligations according to regulations must be fulfilled (two semesters).
c. Students who do not receive written permission from the Faculty Leader and University Leader, then the student is automatically declared to have dropped out of their studies or resigned.
d. Students who are found guilty by the district court and have been sentenced to a criminal sentence, then the student is automatically declared to have lost their rights as a university student and is declared a student to have dropped out of their studies
. e. Regular and independent regular program students (S-1) in the evaluation:
1) Final semester 2 evaluation of
regular and independent regular program students are allowed to continue their studies if at the end of semester 2 they have collected a total of at least 22 credits with a minimum grade of C with the provision of achieving a GPA ≥ 2.00.
2) Final evaluation of semester 4
regular and independent regular program students are allowed to continue their studies if at the end of semester 2 they have collected a total of at least 45 credits with a grade weight of at least C with the provision of achieving a GPA ≥ 2.00.
3) Final evaluation of semester 6
regular and independent regular program students are allowed to continue their studies if at the end of semester 2 they have collected a total of at least 72 credits with a grade weight of at least C with the provision of achieving a GPA ≥ 2.00.
4) Final evaluation of semester 8
regular and independent regular program students are allowed to continue their studies if at the end of semester 4 they have collected a total of at least 96 credits with a grade weight of at least C with the provision of achieving a K ≥ 2.00.
5) Final evaluation of the student's study period is declared to have passed for the Study Program taken at the Faculty

1. Service Applicants come to submit a Student Drop Out (DO) Study Termination Decision Letter by bringing the specified requirements.
2. ULT staff will verify the service files
3. ULT staff will provide a Student Drop Out (DO) Study Termination Decision Letter
4. Service Applicants receive the output
Completion Period: 10 (Ten) working days after the data is verified at the Faculty and sent back for signing.

How to Get a University Accreditation Certificate?

Please, service applicants submit an application for a University Accreditation Certificate to ULT Staff by bringing the following requirements: Original and Photocopy of Last Diploma.

How Do You Get a Certificate of Active Return to Study (AKK)?

c. Letter of Active College signed by the Dean/Vice Dean I.

How to Submit Changes to PDDIKTI Student Personal Data?

Please request service applicants to submit Changes to Personal Data of PPDIKTI Students by bringing the following requirements:
a. Softcopy of diploma,
b. Transcript of grades,
c. Identity Card (KTP),
d. Student Identity Card (KTM),
e. Birth certificate,
f. Family card scanned each file under 500kb.
g. Cover letter from the Dean of the Faculty.

How to Apply for Alumni Validation?

Please, service applicants to apply for Alumni Validation by bringing the following requirements:
